Are we the right practice for you?

Optimed Immunology is a focused practice in adult clinical immunology. The patients best served by this practice are adults with chronic, complex, or unexplained immune-mediated conditions — especially those who feel they have not received clear answers despite previous evaluations.

Send records ahead of your first visit: Fax to (833) 973-5984. Once your appointment is scheduled, you can also upload records securely through the patient portal.

Common reasons patients come to us include:

  • Recurrent sinus, ear, lung, or skin infections that have not been fully explained
  • Suspected primary immunodeficiency (CVID, IgG subclass deficiency, specific antibody deficiency)
  • Hereditary angioedema or recurrent unexplained swelling
  • Severe, eosinophilic, or steroid-dependent asthma
  • Chronic urticaria or angioedema not responsive to antihistamines
  • Suspected mast cell activation syndrome
  • Atopic dermatitis or eosinophilic esophagitis with significant impact
  • Drug allergy that limits treatment options
  • A complex medical history with overlapping immune symptoms

This practice does not provide: pediatric care (we are adult-focused), allergy shots (immunotherapy injections), in-office skin testing, supervised oral food challenges, or primary care services. If those are what you need, please contact your primary care physician for a referral — we are happy to direct you to colleagues who do provide them.

Insurance — what to verify before scheduling

Before your appointment, please call your insurance carrier (or call us at (614) 430-8022 with your insurance card) and confirm:

  • Optimed Immunology / Donald L. McNeil, MD is in-network for your plan
  • Whether your plan requires a referral from your primary care physician
  • Your specialist visit copay and whether you have met your deductible

Do you need a referral?

Some insurance plans require a referral from a primary care physician or a specialist before seeing a clinical immunologist. Please check your individual plan. Even when not strictly required, having your current physician send recent records ahead of your visit substantially improves the value of your first appointment.

What to send before your first visit

The first appointment is much more useful when records are reviewed in advance. The most helpful items to send by fax ((833) 973-5984) or through the patient portal once your appointment is scheduled:

  • Most recent office notes from your primary care physician and any specialists you currently see
  • All prior immune-related lab results (immunoglobulin panels, allergy testing, vaccine response titers, complement studies)
  • Recent CBC and metabolic panel
  • Imaging reports relevant to chronic infections or chronic disease (chest CT, sinus CT)
  • Current medication list including doses
  • Hospital discharge summaries from any admissions in the past few years
  • A short written timeline of your symptoms — one page is plenty

What to bring to your first visit

  • Photo ID
  • Insurance card
  • List of medications and doses (a phone photo is fine)
  • Any records that were not faxed or uploaded ahead of time
  • Notes on your most pressing questions

What to expect during your visit

First appointments at Optimed Immunology typically run 45 to 60 minutes. The visit includes a detailed history, focused physical examination, and a structured discussion of the differential diagnosis based on your situation. Dr. McNeil personally reviews any prior records that have been sent.

By the end of the visit, you will have a clear written plan: what tests are recommended and why, what additional records would be helpful, and what the next step looks like. If treatment is appropriate, the plan also covers timing, prior authorization, and where treatment will be delivered.

What happens after your visit

Lab orders are typically sent to outside labs (Quest, Labcorp, or your preferred laboratory) for convenience. As results come back, they are reviewed by Dr. McNeil personally, and the team contacts you to discuss findings and next steps — either at a follow-up visit, by phone, or through the patient portal. If treatment is initiated, the office handles prior authorization in-house. Patients are not asked to navigate insurance approvals on their own.

Telehealth

Some follow-up visits can be conducted by telehealth depending on the clinical situation, your insurance, and Ohio licensure rules. The first visit is generally in person. If you have a question about telehealth before scheduling, please contact the office.
