Optimed Immunology is a single-provider practice in clinical immunology and allergy, located in Columbus, Ohio (Worthington area). The practice is focused on adult patients with complex immune-mediated disease — primary antibody deficiencies, hereditary angioedema, severe Type 2 inflammatory disease (asthma, urticaria, atopic dermatitis, EoE, CRSwNP), mast cell disorders, and autoinflammatory conditions.
Dr. McNeil is board certified in Allergy & Immunology and Internal Medicine, with over four decades of clinical experience. The practice uses Athena Health as its EMR.
When to refer
Referrals appropriate for this practice include:
- Recurrent infections suggesting possible immunodeficiency — particularly recurrent sinopulmonary infections, bronchiectasis without other cause, or unusual infections in an adult patient
- Patients with low immunoglobulin levels, abnormal lymphocyte subsets, or impaired vaccine response
- Suspected or confirmed CVID, IgG subclass deficiency, specific antibody deficiency, or other primary immunodeficiency
- Recurrent angioedema not responsive to antihistamines — particularly when HAE is on the differential
- Severe asthma requiring biologic therapy consideration, especially with eosinophilic phenotype or oral steroid dependency
- Chronic urticaria refractory to high-dose antihistamines
- Chronic rhinosinusitis with nasal polyps, particularly when biologic therapy is appropriate
- Atopic dermatitis or eosinophilic esophagitis severe enough to consider systemic or biologic therapy
- Suspected mast cell activation syndrome
- Drug allergy de-labeling or evaluation, including patients with multiple antibiotic allergies
- Hypogammaglobulinemia in the setting of B-cell depleting therapy or hematologic disease
When to refer elsewhere
The practice does not provide allergy immunotherapy injections (allergy shots), in-office skin testing, supervised oral food challenges, or pediatric care. Patients who need these services are best served by colleagues equipped to provide them. The office is happy to help direct patients to appropriate resources when this practice is not the right fit.

Including the following with a referral substantially improves the efficiency of the first visit:
- Most recent office notes documenting the relevant clinical concerns
- Recent CBC with differential and complete metabolic panel
- Quantitative immunoglobulins (IgG, IgA, IgM, IgE) if obtained
- IgG subclasses if obtained
- Specific antibody response titers (pneumococcal, tetanus, diphtheria) if performed
- Lymphocyte subset analysis if performed
- Complement studies (C4, C1-inhibitor, CH50) if HAE is on the differential
- Allergy testing results if obtained
- Imaging reports (chest CT, sinus CT) if relevant
- Hospital discharge summaries from recent admissions
- Current medication list including doses
Turnaround
The team responds to referrals within one business day for scheduling. Initial consultations are typically scheduled within several weeks of a complete referral. Consultation notes are returned to the referring physician promptly after the visit. The office is happy to discuss specific clinical questions by phone in advance of a referral.
Co-management
The practice is comfortable with co-management arrangements when appropriate. After consultation, the consultation note specifies which medications and follow-ups will be managed by Optimed Immunology, which the referring physician is asked to continue, and what monitoring or testing the referring physician should track between visits. Communication is direct and ongoing.
